Ministry Leader Database ID Form (For Ministry Leaders who have been in their current position for over a year)

This form is for all ministry leaders who have been in their current job positions for over a year and have never been registered in the OYYACM Database, and would like to receive communications and support from the office. Ministry leaders are those who have been empowered by a Pastor/Director with the role to oversee/coordinate/influence program components of ministry/formation for any middle school (grade 6-8)*, high school (grade 9-12), college age (out of high school 18-21), or young adult age (21+) ministry (including youth-age Confirmation Preparation) at your parish/campus ministry. This includes paid and unpaid directors, associates, coordinators, administrative assistants, "youth ministers", and possibly even PCL's/ DRE's/Faith Formation leaders who have a role of catechesis/Sacramenal preparation for any of these age group. This does not include general core team volunteers, but it may include top-level volunteer who regularly coordinate a ministry program (ex: always coordinates a retreat or mission trip).

This form provides the Diocese of Dallas Office of Youth, Young Adult and Campus Ministries information to help know who we serve and provide the data to best advocate and support your ministry needs.

(*NOTE: In some parish communities, "Middle School" may be 7-8th grade due to the influence of local public school or other parish decision.)

NOTE:
"Director" means you have been empowered to oversee visionary responsibilities (and possible coordination). You can significantly influence the direction of the ministry.

"Coordinator" or "Associate" means that a vision and direction for the ministry is created by another person and you are given a role to coordinate people and the implementation of programs/initiatives to fulfill that vision.

"Administrative Assistant" means you oversee office tasks, but do not interact regularly with volunteers or the population of your ministry.

NOTE: "Age-Group Catechesis" means any planned events with a primary purpose to form/teach teachings of the faith (youth nights, life nights, bible studies, religious education/faith formation sessions, etc.)

"Comprehensive Ministry Programming" means doing any and all events/programs that are beyond primary Catechesis sessions such as evangelization events, retreats, conferences, service projects, socials, praise, worship, & adoration events, or leadership/discipleship formation for those that want to go deeper, etc.
